Conundrum non-solutions

One episode that sticks in my mind is the one where a contestant, given DISCULOUR as the conundrum, incorrectly buzzed in with "discolour". To my surprise, the Wiki tells me that this happened not once, but twice:

Have there been any other occasions on which a contestant has (effectively) just read out what appeared in front of them instead of trying to solve the anagram?

Yes.

The guidelines sent to contestants now explicitly say that the conundrum answer won't be the same as the scramble. So occasionally you get conundrums where the scramble is itself a word, like POLYTHENE or PAROTITIS.

Some comundrums pretty much are spelt out so would they qualify for this thread? The conundrum for the first ever game pretty much was I seem to remember (from watching it recently).
